SUPPORT COORDINATION

Support Coordinators oversee services provided to our clients by DSPD through the home and community based waiver program provided by Medicaid. We help our clients achieve their goals of independence and empower them to live the life they have always dreamed through a Person-Centered approach

CASE MANAGEMENT

Support Coordinators ensure the health and safety of the individuals we serve by conducting regular visits with staff, family and the individuals. We also oversee and advocate for their DSPD budget.
We will always be your advocate!

EDUCATION & OUTREACH

We believe it takes a village! DSPD provides amazing services to those people they serve, but are not all inclusive. Our organization is well versed in additional community resources and always trying to find additional ways to help meet the unique needs of those we serve.
